The Human Subjects Protection Program (HSPP) has developed an investigator's handbook for those investigators conducting research in the social behavioral sciences. The handbook is now available on this website. Click on Investigator Information, then click on Investigators Handbook.
Humans Subjects Protection Program Request for Amendment Form and Study Related Problems Form
This announcement is in regard to the use of the Human Subjects Protection Program Request for Amendment Form and Study Related Problems Form. As of July 1, 2007, please send all requests for study related changes on the HSPP Request for Amendment Form. Additionally, for unanticipated problems involving risks to subjects or others, and serious adverse events, please use the Study Related Problems Form. If you have any questions about the use of either of these forms, please call the HSPP office at 626-6721.
Consent Form Update
The Biomedical Subject’s Consent Form and Parental/Guardian Consent Form templates have been updated. Please utilize the new templates with all new and revised submissions.
Radiation Control Procedures
The Human Subjects Protection Program and Radiation Control Office have put in place and are enforcing procedures for the review of new and amended projects by the Radiation Safety Committee. For any new project involving procedures that expose subjects to radiation or any previously approved project that changes the radiation exposure to subjects, approval by the Radiation Safety Committee must be granted PRIOR to IRB approval. For more information on submission to Radiation Control, please contact Daniel Silvain, Director, Radiation Control Office at 626-5010.
The Human Subjects Protection Program Office Is Moving
The Human Subjects Protection Program (HSPP) office will be MOVING. Our new address is 1235 N. Mountain. We will be located down the alley between Mabel and Helen, just WEST of Mountain Ave. From our old location at Drachman and Vine, follow the arrows on the map to our new location. The HSPP office will CLOSE from March 14 through March 16 to allow for the move.

Consent Form
A new consent form has been posted for consenting participants in social behavioral science research. The form’s language is simpler and is in a question/answer format. This format should now be used for new project submittals. Consent forms for existing projects may be revised using this format and submitted for review and approval, however existing consent instruments may continue to be used.
